Information provided for training
Inquiries and pre-registration forms ask for contact details and relevant player information, such as name, age or birth year, current team, playing level, training goals, preferred dates and group selection. A parent or guardian should provide registration information for a player under 18. Provide only information needed for the request; do not include sensitive medical details in a general inquiry.
Why this information is used
Information is used to respond to requests, assess program fit, organize training, communicate schedules and administer registration. Final registration may request payment and waiver information. Review the final form before submitting it.
Forms, maps and other services
Training and camp forms open on Jotform. Information submitted there is processed through Jotform under its privacy policy. Email and telephone links open your chosen application.
The venue map loads only after you select “Load venue map”. Doing so sends a request to Google, which can receive information such as your IP address and process it under its privacy policy. External links to Instagram, partner websites and directions also take you to services with their own policies. Website fonts are loaded from Google Fonts, so your browser contacts Google when loading those fonts. The hosting service also receives technical information needed to deliver the website.
Access, corrections and privacy concerns
Contact Guido to request access to your information, correct an error, discuss withdrawal of consent or ask about retention and deletion. Identity verification may be needed before personal information is disclosed or changed. Withdrawal may affect the ability to arrange training; applicable record-keeping requirements may still apply.
If a privacy concern remains unresolved, information about further review is available from the Office of the Information and Privacy Commissioner for British Columbia.
